Pediatric Care Membership

We are proud to offer comprehensive care for children up to 17 years for new patients and 21 years of age for established patients. We look forward to continuity of care while building long lasting, trusting, relationships. With unlimited, extended visits, we can and a focus on prevention and wellness, and you wont have to feel like you have to squeeze in all your concerns in a 10 minute visit.

Our mental health membership encourages patients to be seen consistently for follow up and contact us when there is a concern or change without additional visit costs and easy to schedule appointment booking.

  • All major credit cards, debit cards, HSA, ACH (Bank Transfer)

  • If you do not see something that will meet your needs, contact us to discuss alternative options.

  • New patients: Birth- 17 years and established patients until they turn 22 years old.

  • We offer members 10% off services not covered through their membership. We also offer 5% discount for annual payments.

  • Yes. Either party may cancel the membership with a 30 day written notice.

  • We offer ADHD evaluations and consultations for anxiety and depression.

  • No, membership is for follow up appointment for patients that have already been diagnosed.

  • You can pay monthly or annually. Fees are automatically deducted on the first on the month or annual renewal date. We do not prorate memberships or cancellations within the month.
