What is a school or daycare physical?
A school physical is a brief visit to ensures your child meets the school’s minimum requirements and to evaluate for conditions that may interfere with their education or activities. Public schools and daycares require a physical and immunizations and completed state forms upon entry and at specific intervals.
While we are unable to provide shots, we will provide you locations where you can obtain and provide shot records on the required state forms.
